HelloHealth Medical Clinic and Pharmacy is a new clinic that opened at the end of April 2024. They are collecting intake forms for new patients with priority being given to patients who live in the catchment and require medical care.
To register, fill out their intake form at the link (https://form.jotform.com/240994813545263) and they will be in contact. The intake forms include the following:
“Please note: Filling out this form DOES NOT mean you have been accepted as our patient, but we will put you into our patient intake waiting list. Forms will be processed on a first come first served basis. Once received and reviewed, we will be in contact to book your meet and greet appointment.”
