The website of Seymour Health Centre – West 7th indicates that the clinic has a female nurse practitioner who is accepting some new patients. You can submit an intake form on their website, On the homepage, click on ‘Patient Intake Forms’, then “Intake Form” for Dr. Ling Chen 陈医生 – West7th 西7th街.
At the top of the intake form it says: “Please note this is for setting up an initial consultation with a new provider. This does not indicate that the provider has taken you on as a patient. This is a consultation to ensure that you as the patient, and the provider are a good fit for one another for your ongoing healthcare needs.”
NOTE: The clinic’s phone message says they are not accepting new patients, but this seems to be out of date.
